First Visit

Dr. Dana would like to make your first visit go as smoothly as possible. Please arrive 15 minutes early to make sure that your paperwork and insurance information are completed. Your new patient paperwork will be sent digitally before your appointment. Please complete this before your appointment. If you are bringing in multiple new patients, please consider arriving even earlier. If you do not arrive 15 minutes early, you may be asked to be rescheduled. You must bring a copy of your insurance card to this appointment as well as a parent or legal guardian of the child. 

Failure to show to your first new patient appointment will result in dismissal from the practice.
