Insurance & Fees
We do our best to keep this insurance list current, and we also do our best to verify your specific sub-plan with our software prior to your visit. We also highly recommend that you call your insurance, or log on to their website, to verify that Dr. Jonathan Keenan at Holy Family Health is in network and set as your Primary Care Provider (PCP) with your specific sub-plan. Doing all of the above helps to avoid unexpected charges.
Angela Bumpus, ARNP, is currently ONLY seeing self-pay patients, but will eventually be in network with the same plans as Dr. Keenan, below.

*We are not accepting Medicaid. If it’s possible, we recommend considering a subsidized plan on Healthcare.gov or joining a health-sharing plan, some of which are income-based.
Note: You should always check with your Insurance Carrier to verify plan and product coverage as this is not an exhaustive list, and plans change frequently.
Should we not be “in-network” at the time of your visit, you may self-pay, and we can also provide you with a "Superbill" (i.e. a receipt) to submit to your insurance plan for reimbursement. Either way, the cost of the visit is ultimately the responsibility of the patient.

Payment Options
We accept cash, Visa, Mastercard, Discover, American Express, and personal checks.
Card-on-File
We encourage patients to keep a credit card on file to make the checkout process easier, faster, and more efficient. You will no longer receive statements from us, but you will continue to receive your Explanation of Benefits (EOB) from your insurance carrier once your claim has been processed, detailing the charges and payments made on your behalf.
At check-in we will:
- scan the credit card of your choice, including your Flexible Spending Account (FSA) or Health Savings Account (HSA) card
After your insurance has paid their portion, we will:
- notify you via email of the balance owed
- charge the balance owed to your card on file
- email a receipt for the charge
Your credit card information will always be fully protected by our off-site, card-processing partner Elavon, and not on our computers, as required by industry standards (Payment Card Industry Data Security Standard – PCI-DSS).
