How to Request your Medical Records

The easiest way to transfer your records is by logging into your doctor, hospital, or lab's portal and securely sharing your information via "Direct Exchange" or "Direct Address" to jonathan.keenan.1@8042.direct.athenahealth.com Your records will be received instantly and seamlessly integrated into our electronic system.

Alternatively, please follow instructions on https://www.priviahealth.com/medical-records-request/ to get a copy.

You can also upload records on our portal home page by clicking “Attach files”

Or, you can use our Patient Portal by clicking “My Health” then “Forms and Documents” then “Medical Record Request” form. Fill out, Sign, and email that form to medicalrecords@priviahealth.com

For Copies of your Orders and Referrals

Orders and referrals are visible in our patient portal. In the patient portal, click My Health, then Health Reminders. Using the desktop website is the easiest way, or, from your mobile browser, you can select the browser’s menu option to “Request Desktop Site”
