Become a New Patient
We are currently accepting new patients into our practice. Thank you for considering us. To become a new patient:
- Make an Appointment Online
a. To find the soonest appointments, be sure to click the white buttons for "Show Available Appointments" or "View Availability". Angela will usually have sooner availability than Dr. Keenan, but we do open some same day (sick, urgent, or newborn) appointments for Dr. Keenan each morning, which you can book through the website.
b. If you are scheduling for several family members, please choose back to back 15 minute slots. Otherwise your visits will be broken up and the provider will have to see other patients in between your family’s appointments. - Sign up for our patient portal
a. After booking your appointment, navigate to the patient portal from our home page or your confirmation email, and be sure to create your patient portal account or update your information if you already have an account.
b. Prior to your appointment you should receive an email or text reminder link to “Begin Pre-registration.” You can also see this option in your portal under “Upcoming appointments”.
i.Be sure to complete the entire pre-registration process so you can spend more time with your Provider and less time at the front desk checking in. - Download your patient forms online through the patient portal
When you come to our office for the first time as a new patient, we'll ask you to complete some initial forms, including an Authorization and Consent for Treatment form, if you were not able to download them from the patient portal in advance of your appointment.
To make sure there are no delays in care during your first visit experience, please arrive 15 minutes prior to your scheduled appointment to ensure your registration is complete before meeting with your new provider.
Remember to bring:
- Your insurance card
- Valid photo ID
- List of current medications
- Office co-pay
In an effort to respect the time of all of patients, our staff strives to stay on schedule so that other patients do not have to wait.
For patients who are delayed and arrive late for appointment, every effort will be made to see them the same day. However, wait times may apply, or appointments may need to be rescheduled. After 10 minutes a Late Fee of $50 will be applied, and after 20 minutes a No-Show Fee of $100 will be applied.
