Become a Patient – Visit Guide
1. First Visit
We are currently accepting new patients into our practice. Thank you for considering us. To become a new patient:
- Make an Appointment
- Sign up for our patient portal
- Download your patient forms online through the patient portal
Problem Solving
Dr. Patel will examine your history and perform physical exam and spend as much time required to understand your concerns. Then he will order pertaining test (radiology and Lab) to support the diagnosis, establish severity of disease and further planning.
What to bring to your first visit :
- Driver’s license or a valid ID
- Insurance information
- Referral documents, if applicable
- Current medication list
- Completed pre-registration intake forms
- Labs or Radiology reports/CD you may have.
How To Save Time :
When you come to our office for the first time as a new patient, we’ll ask you to complete some initial forms, including an Authorization and Consent for Treatment form, if you were not able to download them from the patient portal in advance of your appointment.
To make sure there are no delays in care during your first visit experience, please arrive 15 minutes prior to your scheduled appointment to ensure your registration is complete before meeting with your new provider.
In an effort to respect the time of all of patients, our staff strives to stay on schedule so that other patients do not have to wait.
For patients who are delayed and arrive late for appointment, every effort will be made to see them the same day. However, wait times may apply, or appointments may need to be rescheduled.
2. Second Visit
Prioritize Counseling
In this visit, Dr. Patel will discuss your test results, diagnosis & severity of disease. Then he will present you with multiple treatment options, risk and benefits. Dr. Patel encourages the questions and answers beyond your satisfactions.
Personalized Care
Like “there’s more than one way to bake a cake” and “all roads lead to Rome”. There are several treatment options available with variable benefits and different risk profile. This is a critical step. Dr Patel’s brings unique minimal invasive procedure options via his extensive experience that minimize risks, optimize patient recovery while achieving maximum benefits. After careful discussion with Dr. Patel and as per your preference, a tailored comprehensive patient care will be formulated to meet the individual needs, preferences risk/benefits ratio.
3. Procedure Visit
Patient Safety
Patient safety is a critical aspect of healthcare delivery and refers to the prevention of errors and adverse effects on patients during the process of healthcare delivery. Prior authorization will be obtained from your insurance company if needed. You will undergo a procedure. Depending on the procedure, usually a short recovery involves and you will be released to home with instruction what to expect. You will receive next day phone call for checking up on your well-being.
4. After Procedure Visit
Immediate post
Depending on your treatment plan or type of procedure, a follow up visit will be scheduled within 1-2 weeks. This is to make sure you have a seamless recovery and to catch any immediate complication.
Follow-up
Depending on your treatment plan or type of procedure, a follow up visit will be scheduled @3months. The purpose of this visit is manifold, continues smooth recovery, to access post procedure improvement after quality of life. You may undergo blood work and/or Imaging to assess radiological response & clinical questionnaires.
