Do you use a CGM or Insulin Pump?

Please read the following directions regarding uploading your data prior to your appointment

Continuous Glucose Monitors (Libre, Dexcom, Medtronic):  

Freestyle Libre: 
          1. Create a LibreView account: https://pat.libreview.io/articles/create-libreview-account
          2. Accept email invitation from DDEC to share data with your provider 
          3. If using a reader, upload your device to LibrewView (https://pat.libreview.io/articles/upload-a-glucose-meter)
                   (NOTE: If you are using a phone, you can skip this step)
          4. Send a portal message to the office MA when you are done, so they can notify your provider to review your data

Dexcom G5/G6: 
          Create a Clarity Share account:

          After you set up your account, create a Clarity Share Code. Send your code to the office via the patient portal​

Insulin Pump Emergency Plan

Like all devices, Insulin Pumps Can Break or Fail 

Know Your Backup Plan and Have What You Need!

As Soon As Possible, Call Your Pump Company And Let Them Know That Your Pump Is Not Working

Be Prepared: 

  1. Have your long-acting (basal) insulin on hand in your refrigerator. Make sure it's not expired and is not opened.
  2. Have the supplies you need to do your injections (ie: syringes / needles) and to check your blood sugar
  3. Know your diabetes:​
    • Latest pump settings
    • Basal insulin rate
    • Insulin : Carb ratios
    • Correction Factors

Will You Be Off Your Pump For Less Than 12 Hours? 

  1. Check your blood sugar before meals and before bed
  2. Take insulin injections with your short acting insulin according to your carb intake at meals and to correct any high sugars​​

Will You Be Off The Pump For More Than 12 Hours?

  1. Check your blood sugar before meals and before bed
  2. Look at your total daily basal insulin --that's the amount of long-acting insulin (lantus, levemir, basaglar, etc) you give
    yourself once daily. If you do not have a long-acting insulin on hand and the office is closed, you may purchase
    NPH insulin from a pharmacy without a prescription. Give yourself 80% of your total basal insulin dose divided into two NPH shots (1 AM and 1 PM)

Ready to Restart your Pump? 

Timing matters! You can restart your pump 26 hours after the last basal (long-acting) insulin injection OR 12 hours after your last NPH injection

When do you need help?

  • Glucose >250 
  • Large urine ketones
  • Glucose not responding to insulin boluses
  • Nausea / Vomiting
