Sports Physicals

Most schools will require a sports physical form to be completed every year. This can be routinely done with your well child exam or you can schedule a separate visit for just the sport's physical. Every school district has their own guidelines, so please check with your school on what they require. Please provide the appropriate form when you come for your visit.
