Become a Patient
Thank you for your interest in joining the practice. In order to prepare for your first visit, we ask that you call the office at 301.865.8333 and our friendly staff will ask you some questions and gather information in order to properly schedule you for your new patient appointment.
If you plan to use insurance for your care, have your insurance card handy during that call. Our staff will collect that information and will pre-verify your coverage before your appointment. Sometimes referrals and/or other pre-authorizations are needed for chiropractic care. We will work with your other providers to help you obtain those prior to your first visit.
Once you have an appointment, our staff will invite you to the patient portal and explain what forms and other information is needed for your appointment. If you do not have internet access or a printer, we will simply have you fill out paper forms in the office.
Please realize that we are a part of Privia Medical Group, so some of the forms and notices you will see on our website are common to all Privia providers, and may not apply to your chiropractic visits. It is best to call our office, and our friendly staff will answer any questions that you may have.
Become a New Patient
We are currently accepting new patients into our practice. Thank you for considering us. To become a new patient:
- Make an Appointment
- Sign up for our patient portal
- Download your patient forms online through the patient portal
When you come to our office for the first time as a new patient, we'll ask you to complete some initial forms, including an Authorization and Consent for Treatment form, if you were not able to download them from the patient portal in advance of your appointment.
To make sure there are no delays in care during your first visit experience, please arrive 15 minutes prior to your scheduled appointment to ensure your registration is complete before meeting with your new provider.
Remember to bring:
- Your insurance card
- Valid photo ID
- List of current medications
- Office co-pay
In an effort to respect the time of all of patients, our staff strives to stay on schedule so that other patients do not have to wait.
For patients who are delayed and arrive late for appointment, every effort will be made to see them the same day. However, wait times may apply, or appointments may need to be rescheduled.
