Office Policies

Code of conduct: We will always treat our patients, families, vendors and other visitors with utmost respect and dignity. We would expect the same from our patients and visitors. Disrespectful language or behavior will result in dismissal from the practice and the premises.

Phone and portal messages: We do our best to return messages promptly. Please be aware that routine, non-urgent messages may be returned within 48 hours. If your question is complex and clinical in nature, then we strongly urge you to schedule an in-person or remote visit. Currently, we are only able to schedule remote visits for our existing patients (not new patients).

Medication refills: We do our best to process the refill requests promptly but please in advance and allow up to 48 hours to process refill requests.

Referrals: We are a consultative practice. We will ask you to obtain a referral from your primary care doctor/provider, even if your insurance does not require it.

Medical Records: Sending a referral and the relevant medical records to us prior to your appointment greatly improves your care. Please ask your referring and/or primary care doctor to send us the most recent office note, a complete medication list, most recent or relevant labs, any pertinent imaging records and a referral with reason for your visit. We greatly appreciate your help.

Payments and financial policy: Please refer to the financial policy

Credit card on file: We require patients to keep a credit card on file to make the checkout/billing process faster, and more efficient. At check-in, we will scan the credit card of your choice, including your Flexible Spending Account (FSA) or Health Savings Account (HSA) card. After your insurance has paid its portion you should receive an explanation of benefit (EOB) with your financial responsibility. We will notify you of the balance owed and charge the balance owed to your card on file and email a receipt for the charge . For your security, your credit card information will always be fully protected by our off-site, secured card-processing partner Elavon and NOT on our computers.

Cancellations: We understand that sometimes events in life would mean that you would need to cancel or reschedule your appointment. Whenever possible, please cancel or reschedule any follow up appointments with a 48-hour notice and new patient appointment with at least a 72-hour notice. This will allow us to appropriately serve other patients who need care. Frequent no-shows/missed appointments may result in discharge from the practice.

Labs: Please try to have any follow up labs that are ordered by us completed at least 4 business days prior to your appointment. This allows us to review and discuss your results during the office. It improves your care.

CGMS/pump linkage: If you are using a continuous glucose monitor such as Dexcom, Freestyle or if you are on an insulin pump such as Medtronic, Omnipod or Tandem then we strongly encourage you to link your device to our clinic through the App. Please contact the office for additional information.

New Patient Policy

  1. Referral: We are a consultative practice. We will ask you to obtain a referral from your primary care doctor/provider, even if your insurance does not require it. We are referral based, specialty practice and we only see patients with Endocrine diagnosis.
  2. Records: Sending a referral and the relevant medical records prior to your appointment with us greatly improves your care. Please ask your referring and/or primary care doctor to send us the most recent office note, most recent or relevant lab, any pertinent imaging records and a referral with reason for your visit. We greatly appreciate this.
  3. Cancellations: If for any reason you need to cancel or reschedule your new patient appointment then please do so with at least a 72-hour notice. We will do our best to reschedule your appointment. However, we would not be able to reschedule those who do not keep their new patient’s appointment without notice (no-shows).
  4. Be prepared: Please complete the new patient packet prior to arriving at the office. You can find it here. Please also bring a complete and full list of current medications and allergies. If you are being seen for Diabetes, then be sure to bring your meter and/or blood sugar log with you.
  5. No-shows for new patient visits will not be rescheduled.
