HOW TO GET RECORDS FROM DR. CLARK’S OFFICE
There are 3 ways to get your records from us:

CURRENT PATIENT. You have a new provider, please send us this info via portal message and we will submit your records to them electronically. If you email us this request, you will need to fill out a records release form before we can send out your records.
CURRENT PATIENT. You DO NOT have a new provider yet, you can order your records from Dr. Kristin Clark's office NOW by going to the Privia Link: https://www.priviahealth.com/medical-records-request/ to start the process. Please read the directions to order your records (Office name is under PRIVIA HEALTH do not use Well Being Medical Care).
PREVIOUS PATIENT, If your last visit was more than 3 years ago. Sometime in January 2025, the official custodian of your records will be Morgan Records Management. To order from Morgan Records Management, there is a nominal processing fee for all medical records requests regardless of who is requesting the records. You may request a copy of your records by visiting www.morganrecordsmanagement.com > Request My Records. They can also be reached by email: medical@morganrecords.com or phone: 833-888-0061 if you have questions.

**WHEN VISITING YOUR NEW PROVIDER, please remind them to change over all the medication list so the new refill requests will go directly to the new provider and not Dr. Clark.**

Practice is Closing on December 4, 2024

I will be UNABLE to provide patient visits or REFILLS after December 4, 2024. I want to give plenty of notice to allow you to find another provider so as not to interrupt your care. I urge you to begin the process of finding a new primary care physician as soon as possible. If you start calling NOW, you should be able to schedule in the near future. If you wait until December, you might not be able to make a new patient appointment until March or even later. Once you have found a new physician, it is up to you whether you transfer your care at once or whether you continue to see me until the practice closes. However, I want to stress that I will be UNABLE TO PROVIDE PATIENT VISITS or REFILLS AFTER December 4, 2024. Also, diagnostic testing including labs and radiology should be done at the latest a week prior to the closing date. It has been a pleasure and privilege for me to have worked together with you in providing your care. I wish you all the best in the future.
