Well Being Medical Care is now closed. This information can also be found on our website www.wellbeingmedicalcare.com. Your portal access with Privia Health will continuously be active, however, you will no longer be able to communicate with Dr. Clark. Any portal issues, please contact Privia Health at 888-774-8428. If you need your medical records or have any questions regarding your records, please contact Morgan Records, your records administrator directly at 833-888-0061 and follow the directions below to retrieve them.

FOR MEDICAL CONCERNS: 

If you do not have a provider yet, consider urgent care for immediate needs. AllCare Ellicott City or First Call Fulton (serves as Primary Care) are nearby urgent cares. Also consider a Privia Health provider for your new primary care because they can see all of your records. FOR MEDICAL RECORDS: 

Order your full medical records from the official custodian of your records, Morgan Records Management, 8 State Street, Nashua, NH 03063. To order from Morgan Records Management, there is a nominal processing fee for all records requests regardless of who is requesting the records. You may request a copy of your records by visiting www.morganrecordsmanagement.com Request My Records. They can also be reached by email: medical@morganrecords.com or phone: 833-888-0061 if you have questions.

This second option for medical records is free. If you had regular visits with Dr. Clark, this would be a great option for your new primary care if you had visits with Dr. Clark between May 2022 to December 2024 go to https://www.priviahealth.com/medical-records-request/ to start the process (Office name is under PRIVIA HEALTH do not use Well Being Medical Care).
