How to Contact Us

With modern communication challenges, we know it may be hard to connect immediately with a live agent. In an effort to expand beyond standard voicemail, we offer several contact options for your convenience.

  • 24/7 Virtual Clinic-patient focused care even when we are closed. Have a non-urgent need but it popped up after hours? Please visit the 24/7 Virtual Care tab to connect with a Privia Provider.
  • Patient requests- the patient portal is the fastest form of communication with our practice. Appointment requests, forms, medication refills, rescheduling or other non-urgent issues can be directed to the appropriate staff member. A patient request form is available via the website tab: patient resources/patient form/additional forms/patient request form.
  • Medical records- established patients have access to their records directly through the patient portal. Formal requests or former AANV patients require completion of a medical record request form. Please send a portal message to admin to have once sent to you. Standard record processing fees apply.
  • Billing inquires- our team can answer general billing questions. For more specific account requests, please contact Privia via the patient support line.
  • Patient support line – if you have any other needs, want to pay a bill, or need assistance with the patient portal, please call 888-909-3029. The Patient Support Team will be happy to help!
