New Patients

Our practice is accepting new patients  for all providers with the exception of Calvin J. Reams, III. It is preferred that you schedule your appointments online. Simply click the link in our website and follow instructions to create a new account and schedule your first visit and any other visits as necessary. As new patients,  please complete all listed fields including all medical information. If you have any issues or questions, or if you prefer to fill out your New Patient packet,  hard copy,  please contact our Phone Receptionist at 229-228-4130, and someone will be glad to help you with portal issues or hard copy documents.
