Dear Patients of Dr. Schwartz,
I hope this letter finds you in good health and good spirits. After much thought and reflection, I have made the decision to retire from my practice as of January 31,2025, after many rewarding years of serving as your primary care physician.
It has been one of the greatest honors of my life to care for you and your families. Over the years, we have shared many moments some joyful, others challenging—and I am deeply grateful for the trust you have placed in me to be part of your healthcare journey.
This decision has not been an easy one, but as I enter this next chapter, I do so with a sense of fulfillment and a heart full of cherished memories. I am confident that the strong relationships we’ve built will continue to inspire me in the years to come.
To ensure a smooth transition of your care, you can transfer your care within our practice to Dr. Hilary Lawless (especially for Medicare patients) or our excellent and wonderful Physician Assistants, Nicole Sibley and Mary Houston. They all share the same commitment to providing compassionate and comprehensive care, and I am confident that you will be in excellent hands. Premier Primary Care Physicians is actively recruiting for another physician to join the team as well.
Your medical records will remain securely with the practice so there will be a seamless transition. If you choose to transfer to a different practice within Privia your medical record will be available to any Privia provider. If you have any concerns or need assistance during this transition, please do not hesitate to reach out to the office.
