Please arrive 10 – 15 minutes early for your appointment for registration.

Kindly remember to bring the following to each appointment:

• Driver’s License or State ID
• Insurance Information
• Preferred Pharmacy Information
• Copies of operation records, medical records, x-rays, MRIs, CT scans from any previous physician or hospital visits
• Medication list (if any)
• Surgery operation report (if any)
