Patient expectations:
- To be respectful of all patients, our team strives to stay on schedule so that other patients with appointments do not experience delays. Therefore, if your needs require more time than permitted with the allotted appointment, we will manage any impending issues and then provide you a subsequent appointment to ensure all your concerns are addressed.
- A no show and late cancellation fee of $50 will be administered if you miss your scheduled appointment or do not cancel within 24 hours.
- For patients who are delayed and arrive late for an appointment, every effort will be made to see them the same day. However, wait times may apply, or appointments may need to be rescheduled.
- Bring your insurance card and a complete list of all medications including supplements to EVERY visit.
- All current and outstanding balances will be collected at the time of service.
- YEARLY annual physical exams and labs are required for every patient.
- Medication refills cannot be sent if you are not current with your follow-up appointments
