Here’s the quickest way to become a new patient:

After your appointment has been scheduled, we kindly ask that you create an online account and enter your information into our Patient Portal PRIOR to your visit in order to make your appointment run smoothly. Please complete the following steps at a minimum:

Upload your insurance card:

On our Patient Portal home page, click “Attach files” and upload pictures of the FRONT and BACK of your Insurance Card and your Driver’s License, so we can expedite insurance verification. You should also click your initials in the top right and update My Profile, Notifications, etc.

AND

E-sign forms:

You are required to E-sign two forms. Click “My Health” on the left, then “Forms and Documents,” then 1) “Preferred Contact and Method of Communication Form” and 2) “FL Authorization and Consent to Treatment Form.” If you have trouble, print the forms, ink-sign, then upload them just like you did for step #1 (or bring the printed and signed forms with you to your appointment).

72 hours before your appointment, you should receive an email reminder link to “Begin Pre-Registration.” Please look out for this email and follow the link to complete a few new sections (Medications, Pharmacies, Social History, etc.). Please pardon the gender questioning– we are working on eliminating these.

Please ensure that you have your insurance card and driver’s license with you and plan to arrive 15 minutes early to your first appointment. If you don’t complete the above steps, please plan to arrive 30 minutes in advance and bring all of your medications with you.

We strongly recommended transferring pertinent medical records to us, including primary care notes, lab results, ER/hospital physician notes, among others.
