At Bohler Family Practice, we are diligent in the prevention and management of chronic diseases such as diabetes, hypertension, high cholesterol, depression and more.
We offer physical exams for all ages, including wellness exams for babies, children, adults, women's health, and men's health. In addition, we provide sports physicals, hearing testing, asthma testing, cholesterol testing, stress testing. We even perform small surgical procedures such as removing moles and simple skin cancers.
From x-rays to ultrasounds, stress tests to bone density tests, we complete all testing on-site so you don't have to make any extra trip
Always Ready To See You
Our schedules aren’t always predictable, are they? Especially when you have kids. Sometimes life looks like juggling a sick child and getting the others to soccer practice on time. So whether you’re able to make an appointment with us ahead of time or not, our staff is ready to see you.
As one of the few offices that offers the convenience of both appointments and walk-ins, we encourage you to swing by if you have a window of free time. You can also call to make an appointment. We have two offices to serve you, one for appointments and one next door for walk-ins.
All You Need In One Place
We do nearly all of our imaging and testing in our office, saving our clients extra trips to other offices and to the city. If you need things like an x-ray, ultrasound, stress test or bone density test, you won’t have to worry about making any other appointments or carving out additional time. You can take care of what you need all in one place.
It's important to us to make your experience with us as seamless as possible.
Minor/Parent Policy
We ask that minor children under 18 years old be accompanied by a parent or legal guardian. We understand that this may be a hardship in some cases, but we believe that only the parent or guardian is able to give us consent to treat a child before and during the visit.
Billing Policy
This office will not mail routine monthly statements. At each visit, you will be asked to pay by cash, personal check, or bank charge card. Delinquent accounts will be on a cash basis only. Insurance claims are filed as a courtesy. You are responsible for your co-pay at the time of the visit. “Check-up” visits, as well as camp, sport, or school physicals, must be paid for at the time of service.
Prescription
To refill a prescription, you must have been seen in the office within the last 6 months. If not, you will need to come for an office visit. All refills should be requested through your pharmacy first. They will contact our office for the refill.
